What are Forms?



Momos Forms is a helpful tool for your business to manage inquiries smoothly. Momos Forms aims to consolidate & categorize all incoming inquiries into the Momos Unified Inbox. This simplifies the process of monitoring, responding to, and tracking inquiries. 


Here's what Momos Forms offers:

  • Integration: It can be easily added to a website or social media. Instead of traditional emails, businesses can use QR codes, deep links, or embedding codes provided by Momos.

  • Customization: You can customize the form fields to suit their needs.

  • Insights: It provides insights into the types of inquiries received and how they're resolved.

  • Workflow Automation: Momos Forms can automate responses or direct inquiries to the right teams, simplifying the workflow.


Types of Forms


Single-page Contact Us Form


The Single-page Contact Us form is customizable for collecting various customer inquiries, such as hiring, partnership, or reservations, providing a streamlined way for customers to reach out.






Winback Form


The Google Review Winback Form helps businesses recover from service issues and collect customer feedback privately. It aims to improve customer satisfaction and enhance business reputation by making the service recovery process seamless.




How to Locate Forms


To find the Forms page, it is located under Care → Tools → Forms


On this page, you can see the dashboard page which provides an overview of the created Forms and their relevant information.




How to Utilize the Forms Dashboard:


Forms that are still active will be on top while inactive forms (with greyed out color) will sit at the bottom of the list. 


Note: Forms that are no longer in use can be deactivated, we don’t allow deletion to safeguard the data.


Active Form: Form that can be accessed and receive submission.


Inactive Form: Form that cannot be accessed.



To deactivate the active forms, click on the Ø icon beside the QR code:





To reactivate the inactive forms, click on the arrow cycle icon:



By clicking on the forms, you will be able to see more details of the form & form submission history:



In the Details Of The Form, you will be able to see:


  • Form’s creation date
  • Form’s status (Active/Inactive)
  • Form’s name
  • Offer: No offer can be attached to a form
  • Template Type (Single Page Contact Us Form/Google Review Winback Form)
  • Submissions: Total number of form submissions





To quickly view your form, you can click the arrow icon at the top:



Or you may copy the URL by clicking on the copy icon:



The search bar can be used to search for form submissions by entering the customer’s name or email address.



By clicking on the forms submitted, you will be directed to the analytic dashboard & access to the form reporting.

By clicking on the QR Code, you get access to the details of the QR code & short URL. Click on the pencil icon to edit your form URL:



If you can’t save the URL (save button being greyed out), it could be the error caused by:

  • Special character error
  • Or the URL is already in use or unavailable (URLs are first come first served basis across all brands, and some might already be taken.)



Click on the download button to save your QR code in PNG or SVG:


Click on the arrow to direct to the URL:
